To schedule an initial appointment, or for more information about our services, please contact us by phone at info@OCHealthPsych.com. Our office staff is happy to answer your questions and direct you to the provider best suited to meet your specific needs.

Once you and your provider confirm an appointment date and time, you will be asked to complete a set of intake forms. Intake forms can be completed and submitted securely online using Patient Studio. Alternatively, you may print the forms, complete them manually, and return them to us by fax or mail. See buttons below for both of these options.
